위궤양 Gastric ulcer
) Dumping syndrome
(1) early dumping
· 식후 30분 이내에 발생
· 50% glucose로 유발가능
· 기전: hyperosmolar gastric content의 rapid emptying
→ ① distention: motility의 자극
② osmotic shift of fluid→ relative hypovolemia
③ vasoactive substance 분비
· vasomotor Sx:
① palpitaion
② tachycardia
③ diaphoresis
④ dizziness
⑤ abdominal discomfort
⑥ vomiting
(2) late dumping
·식후 90분-3시간 발생
·기전: sugar-containing food 가 proximal small intestine으로 rapid emptying
→ insulin의 과다 분비에 의한 rebound hypoglycemia
·Tx:
1) frequent, small meals
2) 단당류 섭취 제한(탄수화물을 줄이고 단백질과 지방을 늘린다.)
3) 식사시 수분양 제한
4) 식사 후 잠시 누워 있는다
5) somatostatin(vasoactive substance, insulin 분비 억제)
→ late dumping Sx 예방
6) 수술: 대부분 도움 안된다.

1) 원인: shock, burn, sepsis, trauma
(1) mucosal ischemia
(2) tissue injury from gastric acid(위산분비 증가는 없다)
